We are a walk-in facility and our average wait time is about 15 minutes. Patients are seen on a first-come, first-served basis, and depending on the severity of the illness or injury. Please understand that the number of patients coming to our clinic at any particular time varies; therefore, you may have to wait a little longer. We always do our best to make sure you are seen as promptly as possible.

Please use the patient portal, MyChart, to keep track of your medical information and view lab results.

Some records may not be on the patient portal. For those, please complete the Authorization for Release of Medical Records form.

For walk-in requests, the Medical Records Department office is located at the Van Wert Health - Hospital (1250 S. Washington St., Van Wert, Ohio). If you need assistance or have any questions, please call 419-238-8684.

Van Wert Health doesn’t issue any birth certificates. Instead, the Van Wert County General Health District maintains all birth records. If you would like your child’s birth certificate, call the health department at 419-238-0808 Ext. 100. You can also request a replacement for a lost certificate at http://vanwertcountyhealth.org/vital-statistics/.

Your child’s Social Security card should arrive in the mail within 3-4 weeks after your baby’s birth. We initiate this process at the hospital, and you don’t need to complete and submit any other documents outside of our hospital as part of this process.
